A New Kind of "Unexplained"

When I have bad news to relay to a patient's family, I sit on it.  All day long it languishes on my "to-call" list until the last minute.  Apparently, REs do the same thing.

We wanted to avoid ICSI unless we really had to and there was no reason to believe we needed it.  I didn't like the increase in abnormalities (albeit very small) and if we could do one thing naturally in this process I wanted to do it.  So we had a semen profile where they went over J's sample with a fine toothed comb for any known issues that would result in a need for ICSI.  He passed with flying colors and we opted not to do ICSI.

All 26 eggs were mature, but only 5 fertilized.  He said there was no reason for it and just something that happens for some couples.  My eggs looked great and the sperm was perfect.  They were all still alive and kicking this morning, but for whatever reason, they just didn't fertilize.  The 5 embryos we do have looked good today, so we're still hopeful, but there probably won't be anything to freeze.  We're going to do a three day transfer on Sunday.

I asked if this was the reason for our infertility and he said that it most likely was.  He said that for that reason alone, having the answers, he thinks we made the right decision to not to ICSI.  I agree wholeheartedly.  And I know, we should be so happy to have 5.  Some people don't get that far, but I'm still in tears.

I spent years of heartbreak and soul searching to find my J.  I hate the feeling that no matter how much we love each other, our gametes don't really like each other much at all.

  2. I'm sorry the fertilization report was so disappointing, but you have 5! That's five little (medical) miracles waiting to happen.

I know how you feel about waiting for the right guy and then finding your you're compatible in every way but one, though. And that sucks.

    Good luck with your transfer!

    1. I like how you added (medical). :)

You've certainly been dealt a rotten hand in the "cruel joke" department. Thanks for the commiseration.

  3. :( I had the opposite problem (ICSI killing my fert rate). It is so crazy and you never can tell what will happen. I am hoping your 5 are all you need. Take care.

  4. Oh dear! I still hope this is your time. I wish there was more of an answer for you and I'm so sorry this is where you're left right now.

  5. Oh honey. :(

Nothing can make it better, but at least you have 5 fertilised? I only got 5 eggs, and we've not heard back yet to see how many came together. That's my biggest worry, too.

Let's pray those 5 little embies are strong, super strong!

    1. Thanks for the glass half full reminder. I'm just hung up on going from 40 follies to 5 embies.

  7. I'm sorry. I know this must be so disappointing. I hope for you, though, that now you have this fertilization issue identified, you and your 5 embies are right on your way to a big BFP.

For what it's worth, we had done split ICSI, where they ICSI'd half my eggs (8) - they all fertilized but virtually none of them made it to day 3, so I had this seemingly good fertilization report but still ended up with only a small number of good quality embies.

I'm thinking positive thoughts for your transfer Sunday. Hugs.

    1. Ugh. Even ICSI isn't a panacea. Will they ever figure this IF crap out?!?!

  8. The 10 strongest of those gametes found each other to make those 5. I hope your baby is among them! Will you transfer 1 or 2? Will they attempt to freeze the rest?

    Good luck on Sunday!

    1. Thanks, Sarah. That's certainly true. We're going to transfer 2. He told me not to expect any to make it to freeze, so I'm trying to be realistic about that.

  11. So glad you got 5!! That is amazing considering if your gametes have a hard time getting along. :( I'm hoping one or two of them stick! Those strong little embryos!!
